Job description

The post holder carries responsibility for the assessment of care needs and their development, implementation, and evaluation of programmes of care for Adults and Older Adults with mental health problems that are referred to the Crisis Resolution Home Treatment Team.

The post holder is responsible on a day-to-day basis to the Clinical Lead but ultimately accountable to the Matron for the overall service. They will be a member of a multi-disciplinary team and work in close cooperation with wards and Primary Health Care professionals. They will also be responsible for maintaining standards of care within the Crisis Resolution Home Treatment service and ensuring an integrated service is provided, fostering a good interdisciplinary working environment.

All permanent staff are enrolled but are not obliged to work additional shifts unless they choose to.

Key responsibilities include:

  1. Establishing and maintaining effective individual health and social care through:
  • Holistic assessment of client needs
  • Planning, implementing, and evaluating interventions
  • Clear documentation of client care
  • Participating in initial assessments and liaising with referring agencies to meet client needs.
  • Engaging in team and clinical meetings, introducing new care models, and participating in research or trials.
  • Being aware of personal values and attitudes to ensure objective, non-judgmental care.
  • Establishing therapeutic relationships with clients and families.
  • Ensuring effective, confidential communication between agencies, including case conferences, record-keeping, and progress reviews.
  • Managing clinical risks through assessment and monitoring of mental state.
  • Identifying and managing clients with behavioural problems, including joint visits if necessary.
  • Developing close links with relatives and carers, especially during hospital admissions and discharges, ensuring follow-up care.
  • Advising on breaches of professional conduct and liaising with managers to ensure policy compliance.
  • Working flexibly across the Crisis Resolution Home Treatment Service within the Trust.
  • Taking on clinical work appropriate to experience and skills, in consultation with supervisors.
  • Contributing to service quality improvements and providing cover for crisis services as needed.
  • Participating in on-call arrangements for the service.
