Overview

To work as part of the Bridport Community Mental Health Team. To be responsible and professionally accountable for a patient caseload within the community. The postholder will be working with people with a serious mental illness, providing high quality nursing care and treatment alongside the multi-disciplinary team.

About the Team

Bridport Community Mental Health Team is a well established multidisciplinary team made up of highly skilled mental health professionals. The team have a compassionate and proactive approach to patient care. The CMHT has strong links with the local community and are at the forefront of an increased number of joint working initiatives with primary care and the voluntary sector. This is an exciting time for Community Mental Health Teams across Dorset Healthcare due to the implementation of the transformation plan.

Responsibilities
  • Be responsible for a caseload in the community, undertaking bio-psychosocial assessments, and providing evidence-based interventions with patients.
  • Cover the duty rota as required.
  • If the postholder is a registered nurse, be rota'd on to the Depot clinic and support the nursing team with taking bloods and be part of managing the Clozaril Clinic.
