A leading healthcare trust in Upwey seeks a mental health nurse to manage a caseload within the community. Responsibilities include conducting assessments, providing interventions, and supporting a multidisciplinary team. Applicants should have experience in mental health nursing and be able to work collaboratively. To work as part of the Bridport Community Mental Health Team. To be responsible and professionally accountable for a patient caseload within the community. The postholder will be working with people with a serious mental illness, providing high quality nursing care and treatment alongside the multi-disciplinary team.
Bridport Community Mental Health Team is a well established multidisciplinary team made up of highly skilled mental health professionals. The team have a compassionate and proactive approach to patient care. The CMHT has strong links with the local community and are at the forefront of an increased number of joint working initiatives with primary care and the voluntary sector. This is an exciting time for Community Mental Health Teams across Dorset Healthcare due to the implementation of the transformation plan.
