Qualifications

• Effective communication skills and ability to work with multidisciplinary teams.

Responsibilities

• Assess care needs for a defined clinical caseload.
• Develop programmes of care and implement/evaluate these programmes.
• Act as a Care Co-ordinator within Care Treatment Planning under the Mental Health (Wales) Measure 2010.
• Work without direct supervision for delegated caseload and liaise with healthcare professionals and statutory/voluntary agencies.
• Travel between sites in a timely manner.
