Job summary

The Community Mental Health Team at Betsi Cadwaladr University Health Board (North Wales) is seeking a Mental Practitioner to work within an integrated, multidisciplinary team, focusing on individuals with severe and enduring mental health problems.

The post holder will manage care coordination and case management for a defined caseload, developing outcome-focused care plans and ensuring risk assessments are completed and documented in line with the Mental Health Measure (Wales) 2010.

Responsibilities

• To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems
• To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure ( Wales) 2010.
• To act as a point of contact for GP's and other interface services to offer advice support and signposting to appropriate agencies.
• Formulate and develop standard and complex outcome focused care and treatment plans.
• Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
