Senior Community Mental Health Practitioner

Location: Ivybridge, PL21 9AB

Salary: £37,338.00 to £44,962.00

Job Description

We are seeking an experienced practitioner to join our Home Treatment Team in Ivybridge. The team covers a large geographical area and collaborates with the West Devon Core Team, sharing management and clinical leadership.

This is an exciting opportunity as the team is expanding its remit to prevent admissions and facilitate early discharges from hospital.

The team is friendly and supportive, with strong leadership. There are opportunities for further training and career development.

Main duties of the job

  • Assess referrals from primary care, psychiatric liaison, community mental health teams, and first response services
  • Work with patients and families to prevent hospital admission and support discharge
  • Lead shifts and plan work across a large area
  • Refer to other services as appropriate
  • Make safeguarding referrals when necessary

Job Responsibilities

Job Purpose

The postholder will deliver high-quality mental health services, managing a caseload with a focus on assessment and treatment for complex cases, including recovery coordination and statutory social care functions. Collaboration with statutory and non-statutory agencies is essential.

The role includes providing supervision, team development, and evaluation.

Qualifications and Experience

  • Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent, with current registration
  • Experience working in inpatient and community mental health settings
  • Experience supervising junior staff and working in multidisciplinary environments

Skills and Attributes

  • Excellent communication and assessment skills
  • Ability to work independently and as part of a team
  • Good planning and organizational skills
  • Knowledge of current legislation and clinical policies

Additional Requirements

  • DBS check and UK registration required
