Job description
Overview

The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will work collaboratively across a range of disciplines to promote a 'needs based' approach to all service users and will promote an integrated approach to service delivery. The role includes developing relapse recognition and prevention plans and sharing these with the MDT, service users and carers where appropriate. The successful applicant will work effectively with adult service users with mental health needs across the CMHT, predominantly in the MDT covering the care coordinator function, and will manage the care of a defined caseload of patients.

Responsibilities
  • Develop meaningful engagement with individuals who are often referred to as \'difficult to engage\' and their families.
  • Work within the CMHT with working-age adults who have a diagnosis of Serious Mental Illness and/or Complex Mental Health Difficulties.
  • Conduct holistic assessments of need and risk, formulate care plans, and deliver treatment plans focused on recovery and maintaining optimal functioning.
  • Collaborate across disciplines to promote an integrated approach to service delivery.
  • Develop relapse recognition and prevention plans and share with the MDT, service users and carers where appropriate.
  • Manage a defined caseload of patients and coordinate care within the care coordinator function.

Pre-employment checks and start date

Start date and onboarding

Recruitment will liaise with you and the hiring manager to arrange a start date. You will be booked onto a Trust Welcome Session and be issued Pennine Care NHS Terms and Conditions.
