A community mental health service provider is seeking a Care Coordinator to work with adults facing severe mental health challenges. The role involves conducting holistic assessments, developing care plans, and collaborating with multi-disciplinary teams to provide integrated care. The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will work collaboratively across a range of disciplines to promote a 'needs based' approach to all service users and will promote an integrated approach to service delivery. The role includes developing relapse recognition and prevention plans and sharing these with the MDT, service users and carers where appropriate. The successful applicant will work effectively with adult service users with mental health needs across the CMHT, predominantly in the MDT covering the care coordinator function, and will manage the care of a defined caseload of patients.
