The post holder is responsible for assessing care needs and developing, implementing, and evaluating care programs for Adults and Older Adults with mental health issues referred to the Crisis Resolution Home Treatment Team.
The post holder reports daily to the Clinical Lead and is ultimately accountable to the Matron for the overall service. They will be part of a multidisciplinary team, collaborating closely with wards and primary healthcare professionals. The role includes maintaining standards of care within the Crisis Resolution Home Treatment service and ensuring an integrated, multidisciplinary approach to client care.
