Job description

The Community Mental Health Team is seeking a skilled, motivated, organized, and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team. The focus will be on individuals with severe and enduring mental health problems. The successful candidate will be responsible for care coordination and case management of a defined caseload of clients with complex and challenging needs, as outlined in Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

Main duties of the job
  1. Coordinate care for a designated caseload of clients with severe and enduring mental health issues.
  2. Ensure adherence to Care and Treatment Plans and compliance with standards set by the Mental Health (Wales) 2010.
  3. Act as a point of contact for GPs and other interface services, providing advice, support, and signposting to appropriate agencies.
  4. Develop standard and complex outcome-focused care and treatment plans.
  5. Complete and document comprehensive risk assessments for actual and potential risks.
  6. Welsh language skills are desirable; applications from Welsh and/or English speakers are equally welcome.
