The Community Mental Health Team is seeking a skilled, motivated, organized, and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team. The focus will be on individuals with severe and enduring mental health problems. The successful candidate will be responsible for care coordination and case management of a defined caseload of clients with complex and challenging needs, as outlined in Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
