We are looking for motivated and enthusiastic Registered Mental Health Nurse, Social Worker or Occupational Therapist with specialist interest and experience in caring for people with Dementia and functional mental health problems to join our CMHTOP. The post is full time.
As a well-established Team with motivated, proactive and supportive team members, we care for service users with complex needs, including severe and enduring mental health needs, dementia and cognitive impairment, requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.
We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Local Authority, voluntary organisations and other stakeholders in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of health and social care.
The Team promotes opportunities for professional development through regular supervision, access to trainings, including managerial and leadership and quality improvement (QI) projects.
Main duties of the job
The role of the post holder will include the responsibility of holding a community based caseload of service users in the capacity as a Care Coordinator, ensuring that their complex needs are proactively assessed, identified, reviewed and appropriate support plan is put in place to meet those needs.
Work closely with PCNs/GPs, Social Services, Housing and other statutory and voluntary agencies to navigate care pathways and ensure that service users benefit from person-centred care and support plan.
Actively engage service users and their carers in all stages of care provision, ensuring that their views and what matters to them are acknowledged and taken into account.
Act as a duty officer of the Team, in line with the Team Single Point of Entry (SPE) duty Rota, processing and undertaking screening and triaging of referrals; and responding to emergencies as they arise.
Undertake preparation and producing of statutory and other reports, and conducting other delegated responsibilities on behalf of the Team.
CPNs wills also be responsible for the administration of depot medication and other clinical interventions. Social Worker will be expected to provide guidance to the Team on social care issues, and practice as an AMHP (with training, if needed). OT will be expected to provide ADL and other relevant assessments within the Team.
Participate in quality improvement initiatives and projects, audits and surveys relevant to the Service.
Make suggestions for any changes necessary for improve Service delivery and contribute to implementing new processes and practices.
To proactively engage in supervision and appraisal and provide supervision to junior colleagues.

Detailed job description and main responsibilities
Education/Qualification/Training
·Qualified Mental Health Nurse, Social Worker or Occupational Therapist with current Registration with Professional Body
·Evidence of Post Registration continuing professional development.
·Higher Education qualification in Mental Health
Experience
·Post qualifying experience at Band 5 and above.
·Previous experience of working with older adults or in community setting.
·Previous experience of functioning as a team member
·Previous experience of independently conducting psycho-social assessment and planning care for service user with complex care needs.
·Previous experience of acting upon service user and carer feedback and information to improve service delivery, taking personal responsibility for resolving the situation.
·Previous experience of willingness to take responsibility and accountability for decisions and actions.
·Previous experience of clinical audits and KPI reporting.
