Are you an experienced Mental Health Nurse looking to work flexibly with the Older Adult Community in High Wycombe? Do you want to join a friendly and supportive multidisciplinary team and have a varied and interesting case load?

We are seeking community mental health nurses to work with our High Wycombe based team. You may possess specialist knowledge of looking after Older Adults, alternatively, you may not have worked with an Older Adult patient group before but will always put the patient at the centre of what you do to provide individualised, person-centred care.

Our hub is based at Saffron House in Easton, High Wycombe, which is the newly established mental health hub in South Buckinghamshire. We provide community-based treatments and are fortunate to work alongside our own dedicated Social Care Team. Our team provide focused interventions for the older adults of South Buckinghamshire.

This role also offers an exceptional support and training opportunities within the team to grow and thrive in your nursing career.

We can accommodate a small number of newly qualified mental health nurses looking for a band 5 developmental/preceptorship post.

This role is eligible for an incentive scheme of £3,500 paid at intervals over an 18-month period. All payments are pro-rata, non-pensionable and subject to a clawback clause. Please note that only employees external to Oxford Health NHS are eligible for this scheme.

Main duties of the job
  • Independently assess patients' mental health needs and develop personalised care plans ensuring that interventions are tailored to individual requirements.
  • Deliver home-based treatment and support for patients in the community experiencing mental health crises, helping to stabilise their condition and support them further.
  • Manage a defined caseload of patients, acting as their key worker. Including continuous assessment, monitoring, and adjustment of care plans to ensure they meet the evolving needs of the patients.
  • Work in partnership with patients, their carers, and other healthcare professionals to set and achieve treatment goals, ensuring care is holistic and patient-centred.
  • Provide various therapeutic interventions in line with professional standards and job specifications, and manage the discharge process, ensuring safe and timely transitions from care.
  • Engage in clinical and managerial supervision, contribute to the training and education of team members, and participate in service improvement initiatives to enhance the quality of care provided.
