Community Mental Health Nurse - Named Worker Role
The post holder will work within a multi-disciplinary service. Core responsibilities will include:
- Acting as a Named Worker
- Comprehensive assessment of new referrals
- Carrying a varied caseload defined by need
- Contributing to a duty system on a rota basis
- Delivering supervision to more junior members of staff
- Contributing to multidisciplinary service development
- Clinical triage and determining the urgency of new referrals to the service
- Carrying out comprehensive assessments of an individual's health and social care needs in various settings and agreeing on the most appropriate course of action
- Assessing, evaluating, and managing clinical risk
- Formulating a care plan based on identified needs
- Delivering evidence-based interventions as appropriate
- Reviewing the effectiveness of care plans at regular intervals
- Working within safeguarding processes, including acting as safeguarding investigator/manager when required
- Collaborating with service users, carers, and partner agencies
- Maintaining accurate and timely computer-held care records
- Undertaking all work in a culturally sensitive manner
- Acting in accordance with Care Management System, Acute, and Schedule Care Pathways as appropriate
- Developing effective working relationships with Primary Care colleagues
