Job description

Community Mental Health Nurse - Named Worker Role

The post holder will work within a multi-disciplinary service. Core responsibilities will include:

  1. Acting as a Named Worker
  2. Comprehensive assessment of new referrals
  3. Carrying a varied caseload defined by need
  4. Contributing to a duty system on a rota basis
  5. Delivering supervision to more junior members of staff
  6. Contributing to multidisciplinary service development
  7. Clinical triage and determining the urgency of new referrals to the service
  8. Carrying out comprehensive assessments of an individual's health and social care needs in various settings and agreeing on the most appropriate course of action
  9. Assessing, evaluating, and managing clinical risk
  10. Formulating a care plan based on identified needs
  11. Delivering evidence-based interventions as appropriate
  12. Reviewing the effectiveness of care plans at regular intervals
  13. Working within safeguarding processes, including acting as safeguarding investigator/manager when required
  14. Collaborating with service users, carers, and partner agencies
  15. Maintaining accurate and timely computer-held care records
  16. Undertaking all work in a culturally sensitive manner
  17. Acting in accordance with Care Management System, Acute, and Schedule Care Pathways as appropriate
  18. Developing effective working relationships with Primary Care colleagues
