We have a great community nursing opportunity within our Intensive Support Team (IST), at High Weald, Lewes and Havens Specialist Older Adults Mental Health service. This post will suit a Registered Mental Health Nurse (we will consider Learning Difficulties nurses who have sufficient experience of working with Dementia or Severe Mental illness)
This post will involve working with older people with complex mental health needs as well as physical health care needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with internal services, and with other partner agencies.
You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users. You will also be responsible for working collaboratively with service users and their carers on to create care plans that meet their needs and maximise their independence.
The post will be based either at Millwood, Uckfield and is 37.5 hours per week and will include weekend working.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 5-6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).
Duties
The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:
To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To develop alongside the service user their care plan.
To offer specialist nursing assessment and advice where required.
To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.
To provide supervision of junior staff and trainees where appropriate.
