Job description

An exciting opportunity has arisen within our Integrated Mental Health Team in Stafford. Integrated Mental Health Teams offer community-based interventions for people experiencing a range of mental health conditions. The teams have been developed in line with MPFT's Community Mental Health Transformation Framework to offer integrated, place-based community mental health services.

We are looking for a Band 6 Community Mental Health Nurse or a Band 5 development post to join our dynamic and friendly Integrated Mental Health Team based in Stafford. The Integrated Mental Health Team is an innovative secondary care community team guided by collective leadership, providing evidence-based psychological and medical interventions to patients with complex mental health needs. This role will focus on mental health needs of a non-psychotic nature (e.g., depression, OCD, anxiety, trauma, personality disorder).

The usual hours of work are Monday to Friday, 9am - 5pm. You will be expected to travel across a large geographical area, so access to suitable transport is essential.

As part of this role, you will be expected to:

  1. In conjunction with the multidisciplinary team, provide nursing interventions to support the person in meeting their goals, and participate in reviewing progress using recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
  2. Participate in setting quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance, practice, and policy.
  3. Administer medication when appropriate, complying with relevant Trust procedures and NMC guidance.
  4. Assess patients' individual care needs, develop, implement, and evaluate care programmes.
  5. Deliver interventions according to the care plan and pathway guidance, including pharmacological and psychologically informed interventions.
  6. Complete and contribute to risk assessments and management plans, promoting positive risk-taking to improve quality of life and independence.
  7. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
