An opportunity has arisen for an experienced, enthusiastic Community Nurse to join our team based at Quay Health Centre, Connah's Quay. The role involves clinic work and community visits.

Main duties of the job

The post holder is responsible for the assessment, planning, implementation, and evaluation of evidence-based nursing care, working collaboratively with others to meet patients' and families' needs.

Assist in managing and organizing the clinical area and participate in the education, development, and supervision of staff members.

Fluency in Welsh is desirable; Welsh and/or English speakers are equally encouraged to apply.
