Job summary

Norfolk and Suffolk NHS Foundation Trust in Norwich is recruiting a Band 5 Community Mental Health Nurse to join the Adult Community North West CMHT on a substantive, full-time basis.

The role involves managing a caseload, providing care in the community, attending MDTs, and delivering medication administration with a supportive, multidisciplinary team. Salary £32,073 - £39,043 per year.

Qualifications

• Nursing degree or equivalent RN(MH).
• Valid registration with NMC.
• Mentorship qualification or willingness to undertake mentorship training.

Responsibilities

• Managing a caseload via face to face and telephone contacts.
• Attending outpatient clinic appointments.
• Undertaking face to face visits in the community including home visits.
• Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders.
• Attending regular MDT meetings and liaising with colleagues from other services.
• Medication administration.
• Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm).
