Job summary

West London NHS Trust is seeking a Band 6 Community Psychiatric Nurse to join the MINT Team in Hounslow on a 6-month secondment. The role involves managing a diverse caseload and working within a multidisciplinary team to provide community mental health support to adults across London Borough of Hounslow.

The successful candidate will have significant post-registration experience, community mental health exposure, and strong assessment and risk management skills.

Qualifications

• Significant post-registration experience working with adults experiencing mental health difficulties.
• At least six months’ community experience working with adults.
• Knowledge of relevant mental health legislation and best practice.
• Experience in mental health assessment, risk assessment, safety planning and medication administration.

Responsibilities

• Develop and maintain communication with individuals or groups of patients, carers and staff, about difficult or complex matters, overcoming any problems in communication in different situations.
• Develop own knowledge and skills and contribute to the development of others.
• Promote, monitor and maintain best practice in health, safety and security of self and others in own work area.
• Appraise, interpret and apply suggestions, recommendations and directives to improve service.
• Contribute to quality improvement.
• Promote people’s equality, diversity and rights.
• Plan, develop and implement approaches to promote health and well-being and prevent adverse effects on health and well-being.
• Assess complex health and well-being needs and develop, monitor and review care-plans to meet those needs.

• Enable people to meet ongoing health and well-being needs.
• Assist in providing interventions and/or treatments.
• Contribute to the assessment of physiological and/or psychological functioning
• Contribute to planning, delivery and monitoring interventions and/or treatments.
• Monitor expenditure.
