NHS Jobs in Rochdale is seeking a dedicated Band 5 practitioner to join a CMHT-based team. You will act as a key worker, manage a caseload, and deliver evidence-based interventions, assessing risk and formulating care plans with the MDT and carers.

You will monitor progress, maintain electronic records, and participate in depot injections when clinically appropriate. You will communicate with service users and families, supervise Band 5s and support workers, and contribute to the duty rota.

Responsabilités

• Act as a key worker and manage a caseload with evidence-based interventions.
• Assess service user and carer needs; formulate and implement care plans with the MDT and carers.
• Monitor and review risk, liaise with agencies, and maintain accurate electronic records.
• Carry out clinic duties including depot injections where nurse-qualified; supervise Band 5s and peers.
• Participate in CMHT duty system; respond rapidly to crises and promote physical health.
