Job description

An exciting opportunity has arisen to help develop and shape a new model of care that breaks down current barriers to care, is non-restrictive, co-produced, culturally sensitive, and accessible. The service will run 24 hours a day, 7 days a week for people residing in the catchment area, offering open access to a range of psycho-social-medical interventions at every stage of their wellness journey. This includes short-term hospitality stays for those in crisis and community-based offers that are embedded in the neighbourhood. We will work in partnership with Look Ahead Housing Association, the Tower Hamlets VCSE Alliance, and experts by experience in a new fully integrated community mental health centre based in Bethnal Green and serving PCN1.


We are looking for a person who is motivated, innovative, and keen to support the development of a new way of working. Integral to the role and ethos of the service is a commitment to co-productivity and the eradication of a “them-and-us” culture, the removal of barriers that hinder good care, and ensuring compassion, empathy, and relationship building as the primary focus.


Responsibilities:

  1. Welcome all attending the community hub within its values and ethos.
  2. Ensure that co-production is at the core of the role.
  3. Ensure that compassion and empathy are central to care delivery.
  4. Consider local demographics for service provision and improvement.
  5. Participate in all MDT working, including meetings, visits, assessments, and interventions.
  6. Make contact with the service user in a timely manner following triage to screen for risk effectively and complete initial assessments.
  7. Be responsible for an identified community caseload, ensuring a high standard of professional conduct to comply with best practice and professional accountabilities.
  8. Promote a service user-focused service.
  9. Participate in audits and surveys relevant to the team.
