LWSW, one of the most exciting organisations in Health & Social Care, is seeking an enthusiastic nurse to join our friendly, dedicated, hard working multi-disciplinary Specialist Outreach Recovery Team.
We are seeking a registered mental health nurse who possesses recent clinical experience in a community or relevant mental health inpatient setting.
SORT work within the personalised care framework with the move towards goal based, individualised care and interventions.
This includes a named key worker for all service users with a clearer multidisciplinary team approach to both assess and meet the needs of service users, to reduce the reliance on services and to increase resilience in systems of care, promote recovery and allowing all staff to make the best use of their skills and qualifications, and drawing on new roles.

Main duties of the jobResponsible to the Manager of the Specialist outreach recovery team for the assessment of needs, development, implementation, and evaluation of care for patients from a specified group of patients. The practitioner is responsible for providing meaningful intervention-based care and delivery of high-quality, safe and meaningful care which helps people to recover and stay well, with documentation and processes that are proportionate and enable the delivery of high-quality care to a defined group of people with a complex mental health problems in a community setting.
To provide caseload management and support to a defined caseload, and also to provide teaching and supervision to students and others undertaking educational programmes.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.
