We are seeking aregistered mental health nurse who possesses recent clinical experience in acommunity or relevant mental health inpatient setting.
SORT work within thepersonalised care framework with the move towards goal based, individualisedcare and interventions.
This includes a namedkey worker for all service users with a clearer multidisciplinary team approachto both assess and meet the needs of service users, to reduce the reliance onservices and to increase resilience in systems of care, promote recovery andallowing all staff to make the best use of their skills and qualifications, anddrawing on new roles.

Responsible to the Manager of the Specialist outreach recovery team forthe assessment of needs, development, implementation, and evaluation of carefor patients from a specified group of patients. The practitioner isresponsible for providing meaningful intervention-basedcare anddelivery of high-quality, safe and meaningful care which helps people torecover and stay well, with documentation and processes that are proportionateand enable the delivery of high-quality care to adefined group of people with a complex mental health problems in a communitysetting.
To provide caseload management and support to adefined caseload, and also to provide teaching and supervision to students andothers undertaking educational programmes.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Liaise withother community resources and facilitate use of mainstream resources.
Maintain acaseload of clients with a significant degree of mental health complexity whoare difficult to engage in services.
Participatein the shift lead and provide appropriate supervision.
Practicewithin the legal and ethical framework set out by mental health relatedlegislation to ensure that service users interests and well-being are at thefore.
Liaise,advise, develop and maintain good relationships with service users, carers,relatives and fellow health, social care professionals who are involved in thedelivery of services.
Participatein all activities aimed at keeping professional skills updated and contributeto dissemination of that knowledge and experience to all staff and studentswithin your area of responsibility.
Act uponreferrals from secondary care, by providing assessment, implementation anddevelopment of programmes of care to individuals, groups and families.
Implementand participate in the formulation of all policy and procedures within the areaof responsibility in consultation with relevant others.
Plan, withteaching establishments, a realistic learning experience for students and forpostgraduate students and be responsible for the quality of the programme.
Participatein and support training programmes designed for all staff groups.
Monitorstandards of practice within the team, ensuring areas of deficit are addressedthrough audit.
Co-ordinatethe collation of evidence within the team in support of standards for betterhealth improving quality.
Independentlyassess, implement and develop programmes of care.
Prioritiseand re-assign team workload and develop caseload management system.
Supervisionand support.
Assist theTeam Manager in the Investigation of complaints made regarding the service.
Identifystaff training needs.
Makejudgements in regard to the quality of the service.
