Job summary

Central and North West London NHS Foundation Trust is seeking a post holder to lead assessment, development and evaluation of care programmes using Dialog+. You will act as a keyworker for a complex caseload within a multidisciplinary team, contributing to specialist assessments and tailored treatments.

You will maintain comprehensive records and deliver both individual and group interventions, in line with personalised care plans and local documentation policies.

Responsibilities

• To assess, develop and evaluate programmes of care using Dialog+ and act as keyworker for a complex case load.
• Work as part of a multi disciplinary team within a team approach.
• Undertake specialist assessment as required.
• Offer both individual and specialist group treatments and to deliver direct care, according to individual treatment plans and in response to identified need.
• Maintain up to date written and electronic records of all interventions, to record information and write reports as necessary.
• To use the integrated mental health services documentation in line with the local policies.

Job description

The post holder will be responsible for the assessment, development and evaluation of programmes of care using Dialog+ and act as keyworker for a complex case load. The individual will be required to work as part of a multi disciplinary team within a team approach. The individual will be required to undertake specialist assessments as part of professional role., To provide mentorship for students on placement within the team.

To manage a caseload of service users with complex needs, using evidenced based / client centred principles to assess, plan, implement and evaluate interventions.

To undertake specialist assessment as required.

To offer both individual and specialist group treatments and to deliver direct care, according to individual treatment plans and in response to identified need.

To maintain up to date written and electronic records of all interventions, to record information and write reports as necessary. To use the integrated mental health services documentation in line with the local policies.
