Job summary

Pennine Care NHS Foundation Trust is seeking a mental health practitioner to assess risks and deliver holistic care for patients with severe and enduring mental health diagnoses. You will carry a caseload, work with evidence-based interventions, and liaise with the MDT to formulate and implement care plans.

The role involves monitoring risks, maintaining records, and potentially administering depot injections in clinics or clients’ homes.

Qualifications

• Experience in mental health practice.
• Current driving license or access to vehicle.

Responsibilities

• Assess needs and formulate care plans according to MDT guidance.
• Implement and monitor care plans and risk assessments.
• Maintain up-to-date electronic records and participate in duty rotas.
• Carry out depot injections in clients' homes or clinics if Nurse-qualified.
• Supervise Community Support Workers and Band 5s as appropriate.
• Liaise with agencies and MDT members to coordinate care.
• Contribute to rapid response for service users in crisis.

Job description

As a mental health practitioner , you will be responsible for assessing potential risks to patients and others. this will involve holistic approach to meet patients needs who have a sever and enduring mental health diagnosis., To act as mental health practitioner, carry a caseload and work with evidence based interventions

• Assess needs
• Formulate care plans
• Implementation of care plans
• Assessment and monitoring of risk
• Monitor, review and evaluate care
• Liaise with all agencies, and members of the multi-disciplinary team
• Carry out clinic duties such as the administration of depot injections either in the clients home or in the clinics if Nurse qualified.
• Supervise Community Support Workers / Band 5s and peers as appropriate
• To maintain accurate, up to date electronic records
• To offer initial mental health assessments
• To participate in the CMHT's duty system on a rota basis. Be able to offer a rapid response to service users' immediate needs, including those arising from crisis.
• To work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex emotional disorders.
• To maintain a persistent approach to engagement where there are complex mental health difficulties.
• To prioritize and work flexibly with MDT in order to respond to rapidly changing clinical needs.
• To promote the physical health and wellbeing.
  RMN: Registered Mental Health Nurse
• Experience in a relevant area of mental health practice. Current driving license or supported driver with use of vehicle.
