The Isle of Dogs Neighborhood Community Mental Health Team is part of the South East Locality catchment are of the London Borough of Tower Hamlets which covers Primary Care Network 7 and 8. We are an established Mental Health team with motivated, proactive and supportive team members. We care for service users with complex needs, including severe and enduring mental health needs requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.
This is an exciting opportunity for the successful applicant to develop their understanding and delivery of Health and Social Care. The successful applicant will work alongside a multi-Disciplinary team and will co-locate with partner organisations to ensure that provision of health and social care is delivered in a coherent and integrated pathway.
The Team promotes opportunities for professional development through regular supervision, access to trainings, including managerial and leadership and quality improvement (QI) projects.

The Isle of Dogs Neighborhood Mental Health Team is a well-established team within East London Foundation Trust and operates as an integrated multi-disciplinary team with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high quality service for people with severe and enduring mental health needs this is inline with Transformation Project. The team actively works in partnership with service users, carers, the Local Authority and with other statutory services and local voluntary groups.

To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
To promote a service user focused service.
Participate in audits and survey relevant to the team.
To participate in development of new policies and procedures as required.
Ensure that clients receive a culturally competent service.
To co-ordinate the day-to-day activity and manage the workload of the Team.
To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to.
To establish and maintain good relationships with service users, carers and voluntary sector colleagues.
To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.
Liaise with General Practice and Primary Care colleagues to promote partnership working.
Ensuring that the local demographics are taken into consideration for service provision and improvement
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
