Band 6 Nurse - North West Sussex Rehabilitation Pathway (Horsham Hub)Are you a passionate and dedicated nurse looking to make a real difference in mental health care? Join our dynamic and innovative team at the West Sussex Rehabilitation Pathway Horsham Hub as a full-time Band 6 Nurse.

The North West Sussex Rehabilitation Pathway provides specialist community and inpatient mental health rehab services for adults over the age of 18, living within North West Sussex. We believe in providing outstanding quality, safe and effective care and treatment that everyone can be proud of.

In this rewarding role, you'll support individuals living with severe and enduring mental health conditions, guiding them through a recovery-focused rehabilitation model. You will hold a caseload of individuals with complex mental health needs, helping them to develop positive coping strategies that foster independence and personal growth.

Working collaboratively with a diverse multidisciplinary team, including psychologists, psychiatrists, occupational therapists, social workers, support workers, and third-sector partners, you will play a key role in providing holistic, person-centred care.

Main duties of the job

The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinicalinterventions relevant to the care group to include education, risk assessments, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students and act as a mentor for student nurses.

Job responsibilities

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.

To develop alongside the service user their care plan.To offer specialist nursing assessment and advice where required.To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.

To contribute to the maintenance and development of the Trust nursing strategy.To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.To provide supervision of junior staff and trainees where appropriate.To act within the NMC code of conduct at all times.

Being a car driver is an essential component to this role, due to travelling to rural area's with poor public transport.
