A leading healthcare provider in London is looking for a skilled Community Mental Health Nurse to improve the well-being of service users. The role involves managing a caseload, conducting assessments, and leading care delivery within a multidisciplinary team. Candidates should have significant experience in mental health and a commitment to high-quality patient care.
We are looking for a motivated, skilled, and experienced Nurse to join our Community Mental Health Services in Bromley West ICMP Team. The post is based at Beckenham Beacon Hospital.
The post holder will be committed to improving the mental and physical well being of mental health service users that are accessing care in our team.
The post holder will be responsible for a caseload of service users who have complex and substantial mental health needs. They will work as a member of the MDT ensuring that high quality individualised care is delivered and that this promotes recovery and maximise independence.
The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs, in accordance with National CPA Guidance.
