Job description

The successful applicant will work as part of the Multi-Disciplinary Team and care coordinate their own caseload of patients in line with the Mental Health (Wales) measure. You will be committed to ensuring the delivery of the highest quality user-centred care and contribute to the development of adult mental health services in Lower Monmouthshire.

The successful applicant will work as part of the Multi-Disciplinary Team and care coordinate their own caseload of patients in line with the Mental Health (Wales) measure. You will be committed to delivering high-quality, user-centred care and contributing to the development of adult mental health services in Lower Monmouthshire.

The applicant will undertake core Community Mental Health Team (CMHT) duties such as routine and urgent assessment clinics, assessing, planning, and implementing care, depot clinics, clozapine clinics, and duty desk responsibilities.
