Job Overview

An opportunity for a keen, enthusiastic nurse to work as part of Bridgend south CMHT. The post is at band 6, working with a multidisciplinary team, acting as the care coordinator for a caseload of patients with varying mental health needs, within the framework of the Mental Health Measures (2010) Wales.

Main Duties

• Manage a caseload of patients with diverse mental health needs and diagnoses.
• Conduct comprehensive assessments, care planning, and implementation.
• Work within team and national standards, including regular reviews.
• Participate in weekend rota (approximately 4 weekends per year, including some Bank Holidays).
