Lancashire and South Cumbria NHS Foundation Trust is seeking a motivated Community Roots Facilitator to work across the Blackpool & Fylde locality, with occasional delivery in the Central locality. You will run face-to-face and digital sessions, build relationships with community partners, and help people access health and wellbeing opportunities.

Responsibilities

• Facilitate face-to-face and digital sessions for learners
• Build relationships with community partners and stakeholders
• Co-produce curriculum content with experts by experience and professionals
• Evaluate and improve course quality based on feedback and developments
• Represent the Community Roots team and contribute to service development
