Job overview
We are looking to recruit two Band 6 Community Mental Health Care Practitioners with a passion for working with people with mental health conditions.
This is a well-established, high-performing team delivering compassionate care in the community to adults with mental health problems. You will work in a diverse, motivated, friendly, agile, flexible, and responsive team.
The post-holder will provide effective and timely care within a multidisciplinary team setting for working-age adults with functional mental health problems residing in the London Borough of Barnet.
You will carry a defined caseload and work closely with team members, including Psychologists, Medical professionals, Social Workers, Nurses, and other Health & Allied professionals to ensure high-quality care.
This work will be underpinned by a bio-psychosocial model of care, and the post holder will ensure that best practice psycho-social interventions are offered within their skill set.
Main duties of the job
- Work as a Care-coordinator in planning, delivering, coordinating, and evaluating assessments and formulation.
- Collaborate with team members, including Psychologists, Medical professionals, and Allied Professionals, to provide optimal care.
- Conduct complex, person-centred assessments.
- Ensure all care is evidence-based, considering physical, cultural, psychological, and spiritual needs.
- Assess care needs, develop care plans, including risk assessments and interventions like Dialog+, and evaluate these with service users and carers.
- Support and mentor less experienced staff, support workers, and students on placement.
- Undertake duties to extend clinical and managerial expertise as delegated by the Team Manager.
- Monitor outcomes of all referrals comprehensively.

Person specification
Qualifications and Training
- Registered Mental Health Practitioner, RNMH/AHP/Occupational Therapist registration.
- Current registration with the appropriate professional body.
- Evidence of ongoing CPD in portfolio.
Previous Experience
- Post-registration experience in a mental health setting.
- Experience working with multiple agencies and professionals.
- Community mental health experience.
- Minimum of 2 years post-qualification experience.
Knowledge and Skills
- Extensive knowledge of evidence-based assessments and interventions.
- Ability to handle difficult situations, including managing aggressive or emotional patients or carers.
- Effective oral and written communication skills.
- High-quality record-keeping, risk assessment, and risk management skills.
