Job summary

NHS Highland is offering a Community Mental Health Assistant Practitioner (Band 4) to support patients in the community within the local Community Mental Health Team in Kyle of Lochalsh. The post focuses on assessment, rehabilitation and relapse prevention to aid recovery.

Qualifications

• Experience in health, social care or mental health settings.
• Good communication and interpersonal skills.
• Understanding of recovery-focused mental health care.
• Ability to work within agreed protocols under supervision of registered staff.
• Driver Licence and use of own vehicle.

Responsibilities

• Supporting assessment of individuals with mental health needs.
• Delivering rehabilitation and recovery-focused interventions.
• Monitoring progress and reporting changes to registered practitioners.
• Supporting relapse prevention and discharge planning.
• Facilitating group interventions and health promotion activities.
• Working with families, carers and partner agencies as part of the multidisciplinary team.
