North East London Foundation Trust has an exciting opportunity for a Community Matron to work within Havering CHSCS Adult services. The role primarily involves in-reach work with our acute partners.

The post involves working as a complex case manager in an acute setting without direct supervision, collaborating with GPs, social services, and third-sector organizations.
You will be responsible for case management, facilitating early hospital discharges, preventing unnecessary admissions, and supporting healthcare closer to home.
The service operates from Monday to Friday.
The post holder must be a Registered Nurse with at least 5 years of community experience managing a complex caseload. Responsibilities include managing patients with complex health and social care needs, working within an integrated care team, liaising with GPs and other agencies, and delivering personalized care. The role involves in-reach work with acute partners and community nursing services.
Although we are a registered sponsor, sponsorship may not be available for all roles. Please check your eligibility under the UKVI points-based system.
This role is subject to a probationary period. Internal applicants are exempt unless they are currently on a probation or bank staff.
All new starters will join on the first Monday of each month and undergo a comprehensive two-week induction, including mandatory training, systems training, and equipment allocation. This induction takes place at our head office in Rainham, Essex.
This role attracts a 15% supplement of the basic salary, with a minimum of £4,551 and a maximum of £5,735 annually.
