Job description
Overview

The post involves liaising with a variety of health & social care providers & other agencies to ensure the provision of high quality care management to this group of patients and their carers. The post-holders will work closely with the Care Group nurse-led unit and community hub located on the University Hospital of Hartlepool site. The service is delivered to a population of patients within the community setting primarily residing in care homes, with the key aim of preventing inappropriate hospital admission/readmission.

The Community Matron service operates over 7 days a week, covering the hours of 08:00-20:00 across the Hartlepool and Stockton localities. The service is a team of highly skilled advanced nurse practitioners working within Care Group 1 of the Trust (Healthy Lives). The matron delivers reactive and proactive interventions to people residing in care homes across the Hartlepool & Stockton CCG area.

Responsibilities
  • Liaise with health & social care providers and other agencies to ensure provision of high quality care management to this group of patients and carers.
  • Work autonomously as an Advanced Practitioner with responsibility for proactively monitoring and managing patients, recognising early symptoms of disease exacerbation, acute illness and injuries based on the understanding of long-term conditions, disease processes, current evidence and practice standards.
  • Deliver pathway-based interventions to people residing in care homes across the Hartlepool & Stockton CCG area.
  • Focus on ensuring people are cared for in their own home environments and that hospital admission is prevented where possible.
