Community Link Worker - West London

A Community Link Worker is required to work on a rolling contract for a charity based in West London.

The Community Link Worker is knowledgeable with their community and the variety of services that are offered there. They have a strong desire to do things correctly and are committed to applying their knowledge to help individuals in their local communities obtain the support they require.

Link Workers strive to ensure that a person's path to improved wellness is as simple as possible by collaborating with clients, medical professionals, volunteers, and service providers.

Responsibilities:

  1. Develop, carry out, and evaluate a wellness action plan that includes one-on-one and group sessions by working with individuals in a coordinated manner.
  2. To evaluate people's requirements, identify results, and ascertain the kind of assistance needed to get past obstacles and accomplish results.
  3. To conduct, evaluate, and revise risk assessments concerning clients, operations, and the environment.
  4. Pointers to current community resources that will help the person reach their wellness objectives, such as healthy lifestyle choices, sports and leisure, the arts and culture, and statutory organisations, as well as to track positive participation.
  5. To determine if a person has maintained their level of well-being and to identify potential risks to doing so, conduct post-program tracking.
  6. To collaborate across the health and social care interface with all other stakeholders, attending pertinent meetings as needed, and to establish rapport and communicate often with social care teams assigned to caseloads.

Experience Required:
  1. Proficiency in providing case management-based interventions for mental health and wellness.
  2. Work experience in primary or secondary health and social care settings.
  3. Knowledge of assisting clients in health and social care settings who face numerous and/or complicated obstacles to achievement.
  4. Capable of exhibiting excellent planning abilities, being well-organised, and meeting predetermined goals.
  5. Knowledge of mental health, learning disabilities, special education needs, and anti-stigma initiatives.
