Role

The Community Link Worker programme in Inverclyde is funded by NHS Greater Glasgow and Clyde. Community Link Workers are based within GP Practices and work across the community, third sector, and public sector to support people in living well by strengthening connections between community resources and primary care.

Community Link Workers provide a service responsive to the needs of a GP practice population, often living in areas affected by socio-economic deprivation. They support patients to identify issues affecting their ability to live well. Using a person-centred approach, the post holder will help individuals identify personal outcomes and priorities, overcome barriers, and connect them to local and national support services and activities.

The Community Link Worker will assist GP practice teams to better match local and national support services to individual needs. They will also build relationships and processes between GP practices, community resources, statutory organisations, health services, and voluntary organisations.
