Job description

The post holder will be based within the North Lincolnshire Learning Disability team and part of the wider Learning Disability and Forensic directorate. Working closely with the Learning Disability Service Manager, the post holder will review current business activities within the service. They will also lead in promoting the service, identifying good practice, and working with the team to implement recommendations.

The post holder will have direct line management responsibilities for the service and ensure robust management of the team.

The community team manager will oversee the day-to-day clinical and operational responsibilities for the clinical services within the nurse teams, the Intensive Support Team, and the wider multi-disciplinary team.

They will lead and support the nursing team in providing high standards of nursing care across various clinical specialties and contribute to the development of community health team services. Additionally, they will lead and embed the principles of transforming care across the learning disability service.

Job Outline
  • Support the service manager in achieving the service's aims and objectives, including deputising and delegated responsibilities.
  • Assist in the effective day-to-day management of the team within available resources and governance frameworks.
  • Review staffing levels and skill mix as necessary.
  • Lead, mentor, and support staff daily.
  • Manage and provide clinical supervision to staff under line management.
  • Work within infection control standards to improve care quality.
  • Enhance environments for patients.
  • Address patient and carer/relative concerns by building strong relationships.
  • Promote partnership working with patients, carers, and stakeholders.
Scope & Authority
  • Manage the team’s daily operations.
  • Develop a culture of person-centred care.
  • Promote an environment respecting equality and diversity, enabling service users and carers to participate actively in care decisions.
  • Manage devolved budgets.
  • Develop communication strategies to improve understanding among staff, patients, relatives, and carers, considering barriers to understanding.
