This is the perfect part-time role for an experienced community learning disability nurse, interested in working in Complex Physical Health (CPH) within the London Borough of Bromley. You will be part of a small team of motivated nurses supporting people with their physical health.
This part-time community nursing position will provide high quality health care to adults with learning disabilities to improve physical health outcomes. You will work within a multi-disciplinary team and will enable people with learning disabilities to access primary and secondary care. You should be enthusiastic and show an understanding of the complex needs of people with learning disabilities.
We offer a range of training and development opportunities both within Oxleas NHS Foundation Trust and externally. You will receive regular supervision and join our learning disability nursing council across three boroughs.
• The post holder will work in the Bromley CTLD Complex Physical Health (CPH) of Learning Disability nursing team, focusing on the additional needs of people with complex physical health needs.
• The post holder will be expected to manage a caseload of adults, working autonomously with a broad spectrum of physical health issues.
• They will provide holistic, high calibre, evidence based, skilled nursing care, assessments, and interventions.
• The post holder will be expected to facilitate a variety of health education and health promotion training and group work, with clients and carers as needed.
• The post holder will support students and junior staff as required and will undertake to provide a rich learning experience for them.
• The post holder will work collaboratively as part of a multi-disciplinary team and in partnership with other agencies/services.
• The post holder will lead on providing robust health liaison to acute and primary care to ensure our service users are able to access mainstream health services.
• The post holder will have a good awareness of the issues surrounding diversity and multi-cultural communities.

To apply national strategies, frameworks and plans to identify to meet the needs of service users in relation to their Physical Health and Wellbeing.
To identify and provide appropriate support to the social well-being of individual service users.
Work alongside the MDT and wider services to address issues to service users accessing services relevant to their individual health and wellbeing needs, as well as their social needs.
Provide health promotion relevant to the individual needs of the service user, and provide support to their support network to more widely promote and improve their health, and prevent avoidable hospital admissions where possible.
Liaise with mainstream services, and advocate the individual needs of the service users to promote their access to these services where required.
Develop own professional skills and knowledge by engaging in continued professional development that are both mandatory and relevant to the role.
