Intensive Support Team (IST)
Band 6 Community Learning Disability Nurse

The Intensive Support Team is a service designed to support individuals with a learning disability who are struggling with mental health crisis and/or behavioural distress, which is placing them at risk of admission to hospital. The team aims to meet the holistic needs of service users and carers, avoiding hospital admission where possible.
We are seeking a nurse with passion and clinical skills to join our innovative Intensive Support Team. The consultant nurse leads a team of dedicated practitioners supporting a caseload of people needing intensive support. The team operates in three locality arms (Spokes), working in partnership with local service users and key agencies to provide vital support, helping people remain safe in the community.
The post holder will be responsible for providing daily clinical and operational support to an all-age intensive support team. The role involves delivering an enhanced model of support throughout the year to serve the most vulnerable in the community at risk of mental health hospital admission. The team also offers in-reach to those admitted to hospital, working with other learning disability teams, leading formulation, training, and reasonable adjustments during hospital stays.
The post involves working as part of a multidisciplinary team of nurses and allied health professionals to reduce avoidable hospital admissions, prevent community care breakdown, and support those in hospital. The post holder will utilize clinical and leadership skills to ensure an evidence-based approach to delivering specialized interventions, including positive behavioral support, mental health assessments, and risk management.
