You'll collaborate with multidisciplinary teams, manage care packages, and provide training, all while accessing a supportive environment focused on professional development and patient care excellence.
As aCommunity Learning Disabilities Nurse, working within our Community Team for People with Learning Disabilities, County Wide; Chippenham, Warminster, Salisbury, Marlborough, in Wiltshire.
This a Community Learning Disability Nursing role which will actively promote the health and wellbeing of people with learning disabilities through a range of specialist clinical and education interventions in line with all the relevant government policies and guidance e.g. Valuing People 2009, Transforming Care 2015 and The Restraint Reduction Network.
General
Collaborate with the multi-disciplinary CTPLD team to achieve health outcomes and contribute to personalized health planning.
Offer specialist advice and training to develop services that address the health needs of adults with learning disabilities.
Perform clinical tasks, procedures, and practices based on specialist expertise.
Manage care packages for individuals with specific funding, such as Continuing Health Care.
Promote learning disability awareness, ensuring reasonable adjustments are made in line with the Equality Act 2010.
Seek nursing supervision and peer support when evidence is not available.
Analyze complex information and act accordingly.
Stay updated on policies from bodies like the NMC, Government, DOH, MENCAP, and BILD.
Engage in ongoing personal development through training and apply new skills in practice.
Follow national frameworks for NHS care, including Care and Treatment Reviews and Transforming Care guidelines.
Provide expert health advice and interventions to service users and their families/carers.
Manage a designated Community Nursing caseload professionally.
Facilitate healthcare for individuals with learning disabilities and collaborate with primary and secondary health teams.
Assess, plan, and evaluate health needs using specialist skills and knowledge.
Support individuals with behaviors of concern through appropriate assessments and interventions.
Ensure informed consent by providing necessary information to clients and carers.
Develop individual care and risk management plans based on the physical and emotional needs of service users.
Promote independence and empower individuals to make informed healthcare decisions.
Respond swiftly to service user needs with minimal supervision.
Collaborate with families and carers to create personalized Care Plans.
Promote healthy lifestyles through education and accessible information.
Utilize effective communication skills to explain complex health issues.
Provide therapeutic group work on issues like assertiveness, relationships, and sexual health.
Training and Development:
Supervise and support Assistant Practitioners and Health Support Workers, conducting appraisals and development reviews.
Mentor learning disability nursing students and Student Nurse Associates during clinical placements.
Collaborate with other specialties to provide care, education, and training to professionals.
Other Factors:
Community-based role with home and day service visits, often requiring travel in rural areas (driver's license essential).
Ability to work independently, manage disruptions in open-plan offices, and concentrate on tasks.
Support service users with long-term or deteriorating conditions, showing empathy and handling emotional situations.
Navigate challenging behavior and mental health difficulties, resolving conflicts as needed.
Flexibility to handle diverse and demanding situations.
