Job description

Are you a Registered Learning Disability Nurse looking to develop your career within our supportive and progressive Learning Disability Community Team based at Leasowe on the Wirral? We offer a 12-month fixed-term contract at band 5 within this team.

You will be a key member of the multi-disciplinary community support team, managing a defined caseload of adult service users with learning disabilities and complex health needs. We seek a caring, compassionate individual committed to delivering high-quality care, ensuring safe practice, and providing a positive patient experience.

Main duties of the job

The post holder will be part of the Community Learning Disability Team (CLDT), working within a multi-professional framework to manage a caseload of adults with learning disabilities and complex health needs, under supervision. Responsibilities include key worker coordination, care planning, risk assessments, and delivering training to families and care providers on challenging behaviours, mental, and physical health. The role involves working flexibly across locations and times to meet service needs.

You will collaborate with clients, families, support workers, and professionals to enhance understanding and responses to challenging behaviours, aligned with PBS principles and the 2014 Positive & Proactive Care guidelines.

Qualifications and experience
  • Registered Learning Disability Nurse (RNLD)
  • Demonstrated commitment to ongoing training and development
Desirable criteria
  • NST 307 / ENB 998, understanding of PBS, functional assessment tools, data interpretation
  • Skills in assessing, planning, and evaluating care
  • Support in crisis situations
  • Record keeping and report writing skills
  • Ability to chair meetings, deliver training, and support clients in crisis
  • Knowledge of legislation such as the Mental Health Act, Mental Capacity Act, DoLs, Mansell reports
  • Interest in Positive Behaviour Support, reducing restrictive practices, person-centred planning, and approaches
  • Experience working with people with learning disabilities in various settings
  • Ability to enhance communication and engagement in skills development and positive activities
  • Experience working within a community team or MDT
  • Experience with complex needs, including challenging behaviour, mental health, forensics, physical health
  • Experience co-facilitating groups

Appropriate UK professional registration is required.
