Job description

Band 6 Community Nurse - Learning Disabilities (12 months Maternity Cover)

An exciting opportunity has arisen for an experienced Community Learning Disability Nurse to work in the Islington Learning Disabilities Partnership (ILDP).

This is a fixed-term contract for a duration of one year to cover maternity leave. We welcome applications from motivated and experienced Band 5 learning disability nurses seeking career progression into a Band 6 role, as well as those looking to gain valuable experience in a community setting.

ILDP is a forward-thinking, fully integrated health and social care community team whose dedicated staff and service providers were awarded the Camden & Islington Team of the Year (2021). We are a well-resourced service passionate about delivering high-quality support to adults with global learning disabilities and their networks. Our multidisciplinary team (MDT) includes psychiatry, psychology, nursing, occupational therapy, speech & language therapy, and physiotherapy. We pride ourselves on high levels of collaboration, working alongside social work and the Community Support Outreach teams.

Our MDT is known for being friendly, inclusive, and sociable. We are located in Islington beside the Arsenal and benefit from our own building with many of our day provisions and residential services nearby.

Main duties of the job

Islington Learning Disabilities Partnership aims to provide accessible, non-stigmatising local services that contribute to the assessment, ongoing care, rehabilitation, and enablement of people with learning disabilities, who may also have physical and/or mental health challenges. The service is committed to Person-Centred approaches, enabling service users to maximise their potential.

The post holder will:

  • Be a member of the ILDP multidisciplinary integrated health and social care team.
  • Participate in initiatives to improve nursing and wider services, such as quality improvement projects.
  • Be accountable for a caseload of individuals with learning disabilities, potentially with complex physical and/or mental health needs.
  • Act as a Care Coordinator.
  • Incorporate health promotion, facilitation, teaching, and service development into your role.
  • Be aware of initiatives and policies embedded into RNLD practice, such as the LeDeR programme and STOMP.
  • Complete continuing health care assessments and reviews as part of the wider MDT.

Job responsibilities

The post holder will be responsible for a defined caseload reflecting local priorities, including:

  • Assessment of care needs; development, implementation, and evaluation of care programs; setting standards of care; liaising with other agencies.
  • Working to reduce health inequalities and improve access to mental health, acute, and primary care services.
  • Providing in-reach services to individuals with learning disabilities admitted to mental health and forensic wards to facilitate discharge.
  • Promoting early intervention to maintain physical and mental health of individuals and their families or carers.
  • Participating in team meetings, supervision, and service development activities.
  • Managing referrals autonomously, prioritising workload, and responding to changing needs.
  • Contributing to nursing assessments within the multidisciplinary team, offering specialist knowledge and skills.
