A community health organization is seeking a Community Learning Disabilities Nurse to promote health checks and work with stakeholders in West Norfolk. The role includes educating GP practices, running health promotion groups, and supporting patients with learning disabilities. Applicants should have a relevant nursing qualification, experience in the community, and strong communication skills. The position offers an annual salary between £38,682 and £46,580, pro rata.
The closing date is 23 October 2025
This is a fixed term contract until 31st March 2026.
This role will be based within the West Norfolk Adult Community Learning Disabilities Team, the main focus of this role is to work with all relevant stakeholders in West Norfolk to promote the uptake and quality of Annual Learning Disability Health Checks.

You will work within an MDT. You will be line managed by the Health Team Manager, and clinically supervised by the B7 Clinical Lead Nurse. You will support and possibly clinically supervise a band 4 Nursing Assistant in the West LD team, based at Park View Resource Centre, Kings Lynn.
You will develop working relationships with other key stakeholders from outside the organisation, including care providers and acute hospitals.
You will communicate with all members of the Community Learning Disability Team and the wider Integrated Care System including acute and Primary Care Networks, to ensure a cohesive approach to care delivery.
You will work in partnership with people with lived experience, including carers and family.
You will work jointly with other professionals within a range of disciplines, including social care.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
