We are recruiting band 6 nurses to work as part of our new Community InReach Team based at Royal Hampshire County Hospital. The successful candidates will have a wealth of experience, skill and knowledge in community nursing or hospital at home care.
The In-Reach Team is a Hampshire and Isle of Wight Healthcare NHS Foundation Trust (HIOWHC NHSFT) Service with community expertise working into Hampshire Hospitals (HHFT) Emergency Department, Assessment Units and In-Patient ward areas to provide specialist community assessment and liaison for patients who require complex assessment and service provision to achieve a safe return to their chosen appropriate environment in a timely manner with full access to the most suitable range of community based services available. They work in partnership with other HHFT teams, HIOW Mental health liaison, Alton Community Hospital (ACH), Community and Social Care services to provide the best outcomes for patients at risk of unnecessary admission or longer length of stay.
Initially these posts are for a fixed term of 6 months and will be based across both Basingstoke and North Hampshire Hospital and Royal Hampshire County Hospital.
Working alongside existing HHFT teams and community colleagues to ensure that where possible, and where safe patients can come into community care pathways as quickly as possible. You'll provide expert community nursing knowledge to support patients and families to develop care plans and be looked after at home. You will also be liaising with the acute frailty team, emergency department and admission units to case manage the needs of patients who are well known to community services tying together all aspects of their care into holistic outcomes.

Qualifications
