Job Description

The role of Community Hospice at Home Senior Staff Nurse/Paramedic is to provide high-quality specialist palliative nursing care, skills, advice, and support to patients, carers, and those important to the patient. This role supports the delivery of planned and unplanned care across Rennie Grove Peace Hospice.

Main duties and responsibilities
  • Participate in developing the planned and unplanned community Hospice at Home model of care.
  • Work as part of a multidisciplinary team to provide individualized patient and family care and support.
  • Be responsible for allocated patients daily.
  • Ensure high standards of clinical practice and patient care.
  • Deliver specialist palliative nursing care, including virtual and in-person reviews in patients' homes.
  • Collaborate with the wider Hospice team and system partners to support care delivery and enable patients to remain in their preferred place of care.
  • Enhance patient outcomes and experience by providing physical and emotional support and coordinating care services.
  • Participate in audits and case reviews to improve the Hospice at Home service.
  • Act as a Rennie Grove Peace ambassador within the community.
  • Support safe and effective service delivery across the geographical area as required.
