Salary: £37,338 - £44,962 per annum (pro rata)

Role Overview

The role of Community Hospice at Home Senior Staff Nurse/Paramedic is to provide high-quality specialist palliative nursing care, skills, advice, and support to patients, carers, and their families. This role supports the delivery of planned and unplanned care across Rennie Grove Peace Hospice.

Main Duties and Responsibilities

1. Participate in developing the organisation's community Hospice at Home care model.
2. Work as part of a multidisciplinary team to deliver individualized patient and family care.
3. Be responsible for allocated patients daily.
4. Maintain high standards of clinical practice and support.
5. Deliver specialist palliative nursing care, including virtual and in-person reviews in patients' homes.
6. Collaborate with the wider hospice team and system partners to support care delivery and enable patients to stay in their preferred place of care.
7. Enhance patient outcomes and experience through physical and emotional support and care coordination.
8. Participate in audits and case reviews to improve the service.
9. Represent Rennie Grove Peace Hospice Care within the community.
10. Support safe and effective service delivery across the geographical area as required.
