Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Community Staff Nurse to join the West Berkshire Community Nursing team. You will support, assess and treat patients in their own homes, delivering high-quality, evidence-based practice with a focus on holistic home care.

The role covers 7 days a week service from 08.30 to 19.00 with opportunities for personal development and career progression. Newly qualified nurses welcome.

Responsibilities

• Holistically assess patients and carers with team to plan, implement and evaluate nursing care.
• Undertake first visits, take immediate action, report back to Team Leader or Community Matron.
• Establish working relationships with other professionals and voluntary agencies.
• Undertake Practice Assessor and Supervisor training and support students and new staff.
