The post holder will be community-based, providing expert clinical knowledge and skills in managing patients with a confirmed diagnosis of Heart Failure. They will manage a specific caseload, focusing on patient management to prevent unplanned hospital admissions, and educate and optimise treatment for newly diagnosed heart failure patients.

They will liaise with the MDT to assess and follow-up with patients, ensuring optimal therapy and promoting self-management education. Coordination with GPs, secondary care, community services, and social services will be essential to develop a multidisciplinary approach to improve patient care and management.

The role involves addressing nursing issues related to Heart Failure in line with NSF standards, local and national guidelines, and influencing strategic planning for Heart Failure service delivery.

Main duties of the job:

  • Manage and facilitate care for Heart Failure patients through outpatient clinics and home visits.
  • Provide specialist assessments and coordinate care plans to maximize health outcomes.
  • Utilize advanced clinical skills and evidence-based knowledge for investigations and treatments.
  • Support medicines management in accordance with guidelines.
  • Offer advice and support to patients and carers, including education and self-management promotion.
  • Coordinate with cardiac rehabilitation and End of Life teams as appropriate.
  • Act as a resource for patients, carers, and health professionals to improve Heart Failure management.
  • Work closely with primary and secondary care to develop integrated care plans involving patients and carers.
  • Ensure mechanisms are in place for escalation during clinical deterioration.
  • Triages referrals from GPs, consultants, inpatient teams, and other professionals.
  • Empower and support patients and carers to actively participate in treatment programs.
