Responsibilities
- Deliver nursing care to patients/carers in homes or clinic settings as delegated by the team coordinator or team leader.
- Assume the extended role of a qualified NVQ level 3 practitioner after appropriate Trust training.
- Communicate effectively and appropriately with all team members and other agencies.
- Undertake delegated duties that facilitate the smooth running of the district nursing team, including clinical procedures such as catheterisation and administration of rectal preparations.
- Provide advice and health promotion to patients/carers as appropriate and undertake clinical observations.
- Obtain specimens as clinically indicated and accurately document patient care in accordance with Trust policy.
- Inform the team coordinator or team leader of changes in the patient’s condition and take delegated responsibility for maintaining stock control and secure storage of stock.
- Undertake non‑direct patient‑related activities as delegated, such as ordering stationery, participating in audit activity, inputting information into the computer system and participating in the identification of personal and professional development needs through team objectives and personal development plans.
- Undertake mandatory training in accordance with Trust policy, understand the responsibility of the role and justify decisions made in practice, actively seek guidance and appropriate learning and development opportunities, report sickness and absence in line with Trust policy, adhere to the Trust policies and procedures, comply with health and safety policies, and ensure prompt recording and reporting of accidents/incidents.
- Organise own daily workload following delegation of daily duties.
- Use effective interpersonal skills when communicating with patients, particularly when the atmosphere is hostile, antagonistic or highly emotive; communicate with members of the primary health care team and multi‑agency to provide high quality holistic care to meet patient needs; disseminate information to multidisciplinary team members, team coordinator or team leader and clinical nurse manager.
- Adhere to Trust policies and procedures and keep informed of the latest Trust news and events.
Qualifications & Key Skills
- Excellent communication skills.
- Ability to work alone.
- Experience of working in community and managing own caseload.
- Experience administering insulin, eyedrops, basic dressings and venepuncture.
- Access to a car with suitable business insurance.
- Hold Level 2 Health & Social Care or NHS Care Certificate (NVQ certificates are not accepted).
- Passion for community care, ability to work effectively within a team environment and independently organise and prioritise workload.

Location: Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh.
Hours: Full‑time role (37.5 hours) to be worked across a 7‑day week.
Car Requirement: As the role involves visiting patient homes & clinics across the Trust every day, postholders must state in their application if they have access to a car with suitable business insurance. If you do not mention this in your application, you will not be considered for the post.
