Community Healthcare Assistant - Adult Community Nursing

We are looking to recruit dynamic, motivated and enthusiastic Band 3 Healthcare Assistant who has a passion for delivering high quality care within a community setting. They will be working within the Havens Primary Care Network (PCN) visiting patients who are registered with GP surgeries; Havens Health in Peacehaven, Chapel Street and Quayside in Newhaven.

This is an ideal opportunity for anyone looking for a career within healthcare, or an existing Band 3 Healthcare Assistant who wants to develop their clinical knowledge and practice within the community. Community and healthcare experience is desirable, but not essential as training will be given to develop the skills and knowledge required for this role such as insulin administration, wound care, completing observations, Catheter care and venepuncture.

Main duties of the job

a) To undertake non-complex clinical procedures under the guidance of senior team members e.g. venepuncture, patient specific sub-cutaneous injections, sample collection, blood pressure monitoring, medications, wound care, catheter care and other tasks according to training and competencies. To recognise their responsibility and accountability in accepting the delegated task.

b) Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team.

c) Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided.

d) To keep accurate, timely records according to Trust policy.

e) To escalate and report patient progress/status on a shift by shift basis to senior team members.

f) To demonstrate flexibility and adaptability working across community teams according to service needs.

g) To commit to undertaking training and education to progress clinical skills as indicated by service development needs.

h) To respond effectively and actively to complaints and escalate appropriately following departmental policies.

i) To assist registered staff in care and treatment and participate in the review process.

j) To promote equality for all, including the maintenance of confidentiality, supporting individual customs, values and spiritual beliefs and ensuring that anti-discriminatory practices are demonstrated.
