A local health provider in Stockport is seeking a Band 3 Support Worker. The role involves assisting with patient care in the community, requiring at least one year of healthcare experience and a valid driving license. The position offers a salary ranging from £24,937 to £26,598 pro rata annually and includes various employee benefits such as annual leave and support schemes.
Our work in Primary Care Networks (PCN) is at the very heart of Stockport PCN Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim is that the "right care is delivered to the right people at the right time".
We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.
Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Where possible, adjustments can be considered in accordance with the Equality Act.
Stockport PCN is in a position to offer exciting new opportunities for Band 3 Support Workers. We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first class service to the residents of Stockport. Line management will be provided by Stockport Foundation Trust, under NHS terms and conditions. On appointment, successful candidates will be expected to complete the Care Certificate.
To work within a multidisciplinary team delivering social and nursing care (appropriate to the post) to clients in their own home. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.
Providing patients with support in administering medications, eye drops, wound care and supporting your colleagues.
Working 24.45 hrs/week between 8am-6pm.

Between 27-33 days of annual leave plus bank holidays
To assist with general wellbeing; creating a supportive and re- enabling environment promoting maximum independence. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients symptom control.
If candidates who are appointed do not hold the Care Certificate, an opportunity to complete will be available. The post holder MUST hold a driving licence with access to a car to fulfil this role.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
