This unique position offers the opportunity to work as part of a highly skilled community workforce, which is the cornerstone of safe patient care. You will collaborate with a multi-disciplinary team to visit patients in their homes and care homes, helping to prevent unnecessary hospital admissions. We provide extensive training and development pathways at all levels, from band 3 to band 8, including a preceptorship program to support your role and ongoing professional growth.

We utilize electronic clinical records via EMIS, and a work iPad and iPhone will be provided.

Main duties of the job

We are seeking a motivated and ambitious healthcare assistant to join our Community Nursing Team, which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates, and Healthcare Assistants.

You will work under the supervision of a Registered Nurse, ensuring compliance with all relevant policies, protocols, guidelines, and SOPs.

The role requires skills in basic wound care, venepuncture, catheter care, etc., and a Level 2 NVQ or equivalent qualification.

Availability to work Monday to Sunday between 8 am and 8 pm is required. A full UK driving license and access to a vehicle for work purposes are preferred; reasonable adjustments will be made for disabled candidates in accordance with the Equality Act 2010.

Job responsibilities

You will deliver care aligned with national and local priorities, policies, procedures, and guidelines, working under the delegation of Registered Nurses within the District Nurse Team. You will perform a range of healthcare activities, including venepuncture, wound dressings, blood glucose monitoring, insulin administration, continence assessments, and stock management, in community settings for patients at home.

Person Specification
Education / Training / Qualifications
  • Evidence of a good level of education
  • NVQ Level 2, Care Certificate, or equivalent, with willingness to undertake further training
  • NVQ Level 3 or equivalent
  • Certificate of competence in adult venepuncture
Knowledge and Experience
  • Knowledge of community healthcare practices, including venepuncture, blood glucose monitoring, stoma and catheter care, and wound care
  • Extensive healthcare assistant experience
Disclosure and Barring Service Check

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a DBS check for any criminal convictions.

Salary: £25,883 to £26,958 per year (Pro Rata) including HCAS
